What Is Transition?

Transition means change. Change happens at various stages of your life. It can be an exciting time, but it can be worrying too for you and your family. The Transition Support Service is here to help.

What does the Transition Support Service do?

If you’re over 16 years of age and have a learning disability, the Transition Support Service will assist you and
your family to make plans for your future.

Making a Transition Plan

The Transition Support Service will work with you and also in partnership with those who are important to you to make a Transition Plan which puts you at the centre of planning for your life.

The Transition Support Service can help you make important decisions about:

*Further education
*Training and Employment
*Housing
*Day care services and Day opportunities
*Retirement
