Our main aim is to enable disabled children and their families to have access to information, assessment and services if required, to help promote their personal development, independence and social inclusion. We work in partnership with children / young people and their families, and with a range of other professionals and agencies, to make a positive difference to the lives of disabled children and their families. We provide support for children 0-18 who have a diagnosed physical or learning disability. This can include respite care, domiciliary care, liaison with other professionals, carers support, support in playgroup. Transition Services. We will complete an assessment of need for all appropriate referrals by talking to you and your child. This will help establish what support you and your family may require, and services may then be offered to help meet these needs.
