Adult Learning Disability Team (Craigavon and Banbridge) - Southern Trust

These Teams are responsible for the multi-disciplinary assessment of need, care planning and review, and referral to relevant support services e.g Domiciliary, day care, respite, day time opportunities, supported living, long term care. These Teams are responsible for the multi-disciplinary assessment of need, care planning and review, and referral to relevant support services e.g Domiciliary, day care, respite, day time opportunities, supported living, long term care. The local Learning Disability Teams within the SHSCT provide integrated health and personal social services to the local adult population who have a diagnosed learning disability. The team also have a carers coordinator.
